The Glucose Challenge Test (GCT), also known as the O'Sullivan Test, is a routine screening test performed during pregnancy, typically between weeks 24 and 28.
Its sole purpose is to screen for Gestational Diabetes Mellitus (GDM) a type of diabetes that develops during pregnancy. Unlike other diabetes tests, this is a preliminary Challenge test to see how your body handles sugar under stress. It is simple, requires drinking a sweet liquid, and does not necessarily require fasting.
Why is this Test Important?
Pregnancy hormones naturally cause insulin resistance. For most women, the body produces extra insulin to handle this. However, some women cannot produce enough insulin, leading to high blood sugar.
If left untreated, Gestational Diabetes can lead to:
-
➤Macrosomia: A very large baby (>9 lbs), leading to difficult delivery or C-section.
-
➤Hypoglycemia in Newborns: The baby's blood sugar may drop dangerously low after birth.
-
➤Pre-eclampsia: High blood pressure in the mother.
How is the Test Performed?
This is a 1-Step Screening Test. It is simpler than the full Tolerance Test (OGTT).
-
1.No Fasting Needed: You can eat your normal meals before the test.
-
2.The Drink: You will be given a sweet liquid containing exactly 50 grams of glucose. You must drink it within 5 minutes.
-
3.The Wait: You must sit quietly for exactly 1 hour. Walking around or eating is not allowed.
-
4.The Draw: After 1 hour, a blood sample is taken to check your sugar level.
Interpreting Your Results
The GCT is a Screening test, meaning it casts a wide net to catch anyone at risk. It does not definitively diagnose diabetes, but it tells doctors who needs further testing.
| 1-Hour Glucose Level | Result Interpretation |
|---|---|
| Below 140 mg/dL (Some doctors use <130 dl="" mg="" td=""> 130> | Normal. You do not have gestational diabetes. No further testing is usually required. |
| Above 140 mg/dL | Abnormal (High). This does NOT mean you have diabetes, but you are at higher risk. You must return for the longer 3-Hour OGTT to confirm. |
| Above 200 mg/dL | High Probability. Often considered diagnostic for Gestational Diabetes without further testing. |
